How Experts Treat Follicular Lymphoma
Season 6, Episode 30
Show notes
Follicular lymphoma (FL) is entering a new era: from decades of chemotherapy to increasingly effective, less toxic biological and immunotherapy-based approaches.
In this podcast episode of EHA Unplugged, hear directly from the authors of the latest EHA guidelines on FL: Prof Marco Ladetto (University of Eastern Piedmont, Italy) and Prof Martin Dreyling (LMU Munich, Germany).
They start by discussing the importance of histological confirmation and grading, the role—and current limits—of molecular profiling, and how FLIPI-based risk assessment can inform treatment intensity.
The conversation covers active surveillance for asymptomatic disease, first-line immunochemotherapy, chemo-free options such as rituximab–lenalidomide and novel antibody-based combinations, and sequencing CAR T-cell therapy and bispecific antibodies in later relapses.
The experts also address survivorship, long-term toxicity, patient preferences, and the strategic need to preserve effective options across a potentially decades-long disease course.
Tune in for practical, evidence-based insights to support individualised follicular lymphoma care.
Guests:
Prof Marco Ladetto
University of Eastern Piedmont and SCDU Ematologia, Alessandria, Italy
Prof Martin Dreyling
Ludwig-Maximilian-University, Munich, Germany

How Experts Treat Chronic Lymphocytic Leukemia (CLL)
Season 6, Episode 29
Show notes
Explore the latest EHA guidelines on chronic lymphocytic leukemia (CLL) and Richter transformation with key guideline authors Prof Barbara Eichhorst, hematologist at University Hospital Cologne (Germany), and Prof Paolo Ghia, Director of the CLL Program at Università Vita-Salute San Raffaele (Milan, Italy).
For the first time, the guidelines no longer recommend chemoimmunotherapy when targeted options are available, based on two key advances: the approval of acalabrutinib plus venetoclax with or without obinutuzumab for frontline CLL, and the approval of pirtobrutinib, a non-covalent BTK inhibitor, for relapsed/refractory disease—particularly after prior covalent BTK inhibitor treatment. The guests also make clear that patient selection matters, including TP53 status, IGHV status, comorbidities, co-medications, safety profile, and patient preferences.
Further, the experts highlight a landmark change in how the recommendations were developed: for the first time, patients and patient advocates participated as authors, bringing a patient-centred perspective to treatment decisions, active surveillance, and supportive care.
Tune in for practical insights into personalized CLL management, fixed-duration targeted therapy, treatment sequencing, and the challenges of Richter transformation.
Guests:
Prof Barbara Eichhorst

Getting Patients to CAR T: Referral Systems in Europe
Season 6, Episode 17
Show notes
In this episode of EHA Unplugged, Prof Marion Subklewe discusses one major challenge facing CAR T-cell therapy in Europe: ensuring that eligible patients are referred for treatment. She explains how access has improved since the first CAR T-cell approvals, but highlights that many eligible patients still do not receive these potentially curative therapies. The discussion explores differences between European healthcare systems, barriers to referral, and persistent misconceptions about patient eligibility.
Prof Subklewe advocates for structured referral pathways and multidisciplinary boards that assess all eligible patients in collaboration with CAR T-cell experts. She argues that treatment eligibility should not depend solely on individual physicians and highlights the benefits of coordinated networks, shared decision-making, professional education, and patient engagement.
Listen now for insights into how stronger referral systems could help more patients access CAR T-cell therapy across Europe.
