  1. 043: Confidence, Discounting, and Provider Revenue

    Show notes

A full schedule can look great on paper and still leave a provider underperforming.

Janelle and Rebekah break down why two providers can both be fully booked while producing completely different revenue. The difference often comes down to timing, appointment mix, treatment planning, confidence, and whether the provider is actually making the strongest recommendation for the patient in front of them.

They also dig into the balance between productivity and patient care. Follow-ups, consultations, and lower-revenue services still matter because they build trust, improve outcomes, and help create long-term patients. The goal is not to turn every appointment into the biggest ticket possible. It is to use the schedule strategically while still delivering the right care.

From under-dosing and thinking with the patient's wallet to over-discounting because you are afraid they will leave, a lot of these habits come back to confidence. Strong providers know their worth, give patients their full range of options, and let the patient decide.

When the focus stays on the best possible outcome, better retention, stronger trust, and healthier revenue usually follow.

  2. 042: Why Your Leads Aren't Booking

    Show notes

A med spa can have plenty of leads and still struggle to fill the schedule. The problem is that "lead" often gets reduced to someone who filled out a digital form, when opportunities are showing up everywhere: patients already sitting in the treatment room, callers asking about services, and people responding to paid campaigns.

Rebekah and Janelle break down where those opportunities are getting lost. Rebooking starts with the provider setting up the next step, not simply leaving it to the front desk. Phone conversions depend on a team that can guide the conversation, credential the practice, answer questions with confidence, and give potential patients a reason to choose you over the next clinic they call.

The same thinking applies to digital leads. More leads mean very little when the conversations behind them are weak. The real opportunity is understanding why people are not booking, training around those gaps, and turning the leads already in front of you into appointments.

  3. 041: The Advice We Wish We'd Taken Sooner

    Show notes

Janelle and Rebekah look back on the careers they started in their 20s and talk through what they would do differently with the experience, confidence, and perspective they have now.

They reflect on the paths they took, the lanes they wish they had found sooner, the leadership lessons that took time to learn, and the mistakes that shaped how they show up today. Rebekah talks about becoming clearer on her love for marketing and learning the balance between relational equity and professionalism. Janelle shares why she would have asked more questions, leaned into leadership earlier, and trusted her own voice sooner.

The conversation also gets into boundaries, burnout, confidence, toxic leadership, staying too long in places that no longer support growth, and learning not to attach self-worth to external praise. It is a reminder that growth often comes from discomfort, self-reflection, and the willingness to keep learning from the past without staying stuck in it.

  4. 040: The Hardest Conversations We've Ever Had

    Show notes

Fun leadership moments are easy to talk about. The hard ones are the conversations nobody is excited to have, but every strong med spa leader eventually has to learn how to navigate.

Janelle and Rebekah talk through some of the toughest workplace conversations they have faced, from hygiene and professional presentation to difficult feedback, hurt feelings, communication breakdowns, and moments where someone had to hold a mirror up to them. These are the kinds of situations that can feel awkward, personal, or uncomfortable, especially when the goal is to protect both the person and the team.

The heart of the message is simple: tough conversations should still be human. Strong leadership is not about coming in hot, proving a point, or hiding behind a title. It is about understanding the why, knowing the person in front of you, creating enough trust for honest feedback, and using brave communication to build a stronger team.

  5. 039: Behind the Botox: The Reality Show Edition

    Show notes

Janelle and Rebekah imagine what a med spa reality show would look like, built from years of behind-the-scenes experience in clinics, operations, patients, teams, and all the personalities that come with the industry.

They talk through the cast of characters every med spa team would recognize: the pot stirrer, the mother hen, the diva injector, the exhausted manager, the marketing maven, the quiet one in the corner, the person whose personal life is always on fire, and the generational mix that makes clinic culture even more entertaining.

They also get into the patient side of the show, from the regular who feels like one of the girls to the demanding VIP, the event guest who comes for the snacks, and the patient whose confidence transformation reminds everyone why the work matters. It is funny, chaotic, familiar, and just real enough that anyone who has worked in a med spa will probably start casting their own clinic immediately.

  6. 038: If We Were Handed a Med Spa Tomorrow

    Show notes

Operation Med Spa welcomes its first studio guest, Jessica, one of Alpha's Directors of Operations, for a practical conversation about what they would do if someone handed them the keys to a med spa tomorrow.

Rebekah, Janelle, and Jessica talk through the first moves that matter most: observing the business, meeting with every employee, understanding the patient experience, reviewing the P&L, walking the neighborhood, evaluating the space, and identifying what is actually driving the business before making big changes. The goal is not to rush in with a resume and start changing everything. It is to understand the clinic from every angle.

They also break down staffing, culture, vendor relationships, service selection, device graveyards, quick wins, facility updates, retail, CRM, marketing, and whether they would keep or change the business name. The conversation shows the difference between thinking like a director and thinking like an owner, while still protecting the team, the patients, and the long-term health of the business.

  7. 037: The Dark Side of Med Spas

    Show notes

Not every med spa experience is sunshine and rainbows. Janelle and Rebekah talk through the patient-facing red flags that can show up in the industry, from misunderstood Botox dilution to illegally sourced product, expired retail, questionable sanitation, and pricing that starts to feel more like a negotiation than a medical aesthetics consultation.

They explain why seeing liquid added to Botox is not automatically a red flag, but pricing that seems too good to be true might be. Extremely cheap injectables can point to bigger concerns around sourcing, expiration dates, storage, or whether the product came from a certified vendor.

The conversation also covers retail products, open syringes, treatment room cleanliness, and why trustworthy med spas should operate with consistency, safety, transparency, and integrity. The goal is not to scare patients. It is to help them ask better questions, notice warning signs, and choose providers who care about doing things the right way.

  8. 036: Why Teams Stop Respecting Leadership

    Show notes

Janelle and Rebekah talk through one of the most important parts of leadership: respect. Respect is not usually lost in one big moment. It erodes slowly through avoided conversations, inconsistent standards, favoritism, reactive leadership, lack of follow-through, and leaders who care more about being liked than being effective.

They break down why teams notice what leaders tolerate, why hard conversations matter, and how inconsistency can quietly damage trust. If one person gets away with behavior that someone else would be coached for, the team sees it. If meetings happen but nothing changes afterward, the team feels it. If leaders avoid accountability because they want everyone to like them, respect starts to disappear.

The conversation also answers the bigger question: can respect be earned back? Janelle and Rebekah agree that it can, but only with honesty, self-awareness, humility, clear communication, and consistent action over time.

  9. 035: The Five Delusions of Med Spas

    Show notes

Janelle and Rebekah break down five common med spa delusions that can quietly hold a business back: "we just need more leads," "our schedule is full, so we're doing great," "our culture is amazing," "operations isn't that hard," and "that's not my job."

They unpack why more leads do not matter if response times are slow, follow-up is weak, or the team is not converting inquiries into booked appointments. They also explain why a full schedule does not always mean a healthy business if revenue per visit is low, discounts are heavy, cancellations are high, or the team is not protecting the book.

The conversation also gets into culture, accountability, operations, and teamwork. A nice team is not always a healthy team, and strong med spa culture requires direct feedback, ownership, and people who are willing to jump in even when something is technically not their job.

  10. 034: Why Bad Inventory Hurts Your Bottom Line

    Show notes

Inventory may not be the most glamorous topic in med spa operations, but it is one of the most important. Rebekah and Janelle explain why inventory directly impacts both top line and bottom line performance, especially when it comes to injectables, retail, laser tips, and other consumables tied to revenue.

They break down the dangers of overstocking, understocking, buying based on vendor excitement, failing to rotate product, and letting expired inventory turn into lost revenue. They also explain why practices need to know their run rates, maintain healthy par levels, and take inventory tracking seriously before small losses become major financial problems.

Strong inventory systems protect profit, improve patient experience, and build trust across the team. From communication slips and daily counts to manager sign-offs and retail tracking, every process helps prevent avoidable loss and keeps the business running with more accuracy and control.

  11. 033: Rethinking How You Book New Clients

    Show notes

Janelle and Rebekah take on one of the more uncomfortable scheduling questions in med spa leadership: should new consults automatically be distributed evenly, or should provider rebooking rates influence where those new patients go?

They break down why the traditional idea of "balancing the books" can miss the bigger business picture. New patients are expensive to acquire, and if a provider is not consistently rebooking or retaining them, the practice may be losing both marketing dollars and long-term patient value. But this is not about cutting providers off without warning. It is about setting clear expectations, coaching intentionally, tracking trends, and helping providers understand why rebooking matters.

A people-first approach can still protect the business. Providers need support, shadowing, consultation training, and honest conversations, but practices also need to be thoughtful about where they place new consults. Rebooking is not just a number. It reflects patient trust, provider development, long-term growth, and the health of the business.

  12. 032: Plastic Surgery Is Having a Moment

    Show notes

Plastic surgery is getting more attention than ever, but Rebekah and Janelle do not see it as a short-term trend. They see a category gaining momentum as techniques improve, results look more natural, and patients become more open about what they are doing.

Janelle shares her recent quad blepharoplasty and earlobe reduction experience, while Rebekah reflects on her own upper blepharoplasty and the fears that came with wanting to look refreshed without looking different. They talk about why before and after photos matter, why patient education matters, and why the consultation, pre-care, post-care, and follow-up experience can make or break trust.

They also connect the conversation back to med spas and plastic surgery practices. The takeaway is simple: patients do not just want skill. They want clarity, connection, honesty, and care from the first call through recovery.

  13. 031: One Provider, One Point of Failure

    Show notes

Janelle and Rebekah take on a question many med spa owners face when growth starts to depend on one major producer: is building a practice around a single high-performing provider smart or dangerous?

They unpack both sides of the decision, from the financial upside of starting with a strong book of business to the very real risks of culture issues, power shifts, favoritism, provider burnout, life changes, and unrealistic revenue projections. A top provider can create momentum, reduce ramp time, and help a new location grow faster, but only if the right expectations, leadership, and safety rails are in place.

Janelle and Rebekah also explore what separates a provider who strengthens the whole team from one who becomes too central to the business. The takeaway is clear: the strategy can work, but only when the practice is built with balance, protection, and long-term stability in mind.

Not everybody can be the best. Otherwise it wouldn't be called the best. So what do the top med spas actually have in common?

Rebekah and Janelle get into the top seven things they keep seeing across the strongest performing practices. A clear vision that goes beyond money and gives the whole team a true north. Goals that are reverse engineered all the way down to patients per day, average ticket, and revenue per hour. Data that tells a story, not just a number you check and move on from.

A service mix that protects your bottom line, not just your top line. Hiring with real criteria and firing fast when it is not working. Training that builds confidence at every level, from the front desk to the provider chair. And attitude, the servant leadership mentality where nothing is above anyone, that shows up across every great practice without exception.

They unpack when discounts actually help your business grow and when they quietly destroy your margins, brand, and patient behavior. They also explore what makes a discount strategic. From using promotions to change patient behavior to creating intentional marketing calendars, they share how to approach discounts with purpose instead of reaction. By the end, they reframe discounts as neither good nor bad, but something that must be used carefully. When done right, they can drive growth. When done wrong, they can slowly undermine your entire business.

  19. 025: Why Combination Treatments Are the Future of Med Spas

    Show notes

Rebekah and Janelle dive into one of the biggest shifts happening in med spas right now, how to deliver better results in less time. As patients become busier and less willing to book multiple visits, the traditional step by step treatment plan is starting to fall short.

In this episode, they break down why combination therapies are becoming essential, not optional. From pairing injectables and devices to bundling services into solution driven offerings, they explain how combining treatments helps patients get where they want faster while improving satisfaction and loyalty.

    Show notes

Rebekah and Janelle tackle one of the most sensitive topics in the med spa industry: price increases. As practices head into a new year, many leaders know adjustments are necessary but hesitate out of fear. In this episode, they walk through the real reasons price increases matter, when to implement them, and how to do it without damaging trust or retention.

Drawing from firsthand experience running practices and leading teams, they break down why holding prices flat erodes margins, limits growth, and ultimately impacts patient experience. They also challenge the idea that being the cheapest creates a competitive edge and explain why premium pricing attracts the right long term patients.

The conversation covers cadence, competitive analysis, communication strategies, and how to prepare staff to handle questions with confidence. If pricing decisions have been holding you back, this episode provides a clear, practical framework for moving forward.
