Weight regain on a GLP-1 can happen even while you’re still taking the medication. So what is actually going on?
Susan Peirce Thompson, PhD, a brain and cognitive scientist who studies eating behavior, addiction and weight regulation, joins Kim and Kat to unpack the biology behind regain, from leptin and hunger signaling to food noise, addiction and the brain’s drive to defend body weight.
They also get into a harder question: where do nutrition and behavior fit when obesity is a chronic disease treated with medication?
And this conversation isn’t finished. Susan is coming back for Part 2, so subscribe to The Plus SideZ Podcast so you don’t miss it.

If you’re in your 40s and have finally realized perimenopause may explain what’s happening in your body, the next question is usually: what does HRT actually look like in real life? What do you take, how do you use it, where do estrogen, progesterone and testosterone for women fit in, and what are the risks?
Kim and Kat are joined by Dr. Patrick Taylor, board-certified family physician, menopause specialist, and Chief Medical Officer at Belle, along with Tiffany Bailey, PA-C, Head of Patient Education at Belle, for a candid deep dive into hormone replacement therapy, the myths women have been carrying for decades, and what treatment can actually look like from one woman to the next.
We get into:
• Do you really need blood work before starting HRT?
• Estrogen patch, pill, cream, or injection: what’s the difference?
• Testosterone for women: when is it considered during perimenopause and HRT?
• What is vaginal estrogen, and why are so many women just now hearing about it?
• What are the real risks of HRT, including breast cancer and blood clots?
• How did the Women’s Health Initiative shape what women were told about hormone therapy? If you’re past “what is perimenopause?” and ready to understand what HRT actually looks like in practice, this episode is for you.

In this episode of The Plus SideZ Podcast, Dr. Pat shares his personal experience with obesity, weight stigma, body image and the mental load of constantly thinking about food. After surviving cancer treatment as a child, he developed Cushing’s syndrome and gained significant weight, eventually reaching nearly 300 pounds before focusing on his health and losing more than 100 pounds.Belle
But weight loss did not erase every part of living with obesity. Dr. Pat talks about why he later chose to use a GLP-1, what changed with his food noise, and how his own experience influences the way he approaches patients today. We also get into GLP-1 plateaus, metabolic adaptation, regain and what may be happening when a medication does not feel as effective as it once did.

Whether you’re on Medicare yourself or helping someone you love navigate GLP-1 coverage, this episode is worth a listen.
In Part 2 of our Medicare GLP-1 Bridge conversation, Dee Dee returns to share what happened after our first episode, and Kim’s mom, Debbie, joins to tell her own story of going from denial to approval. They were both forced to go to compound Tirzepatide when Medicare stopped covering their GLP-1 a couple of years ago.
They walk through the real process of trying to get GLP-1 coverage for obesity, including Medicare Part D, prior authorization, pharmacy rejections, insurance denials, phone calls, paperwork, and the steps that finally moved their approvals forward.
One of the biggest lessons? A rejection at the pharmacy is not always the same thing as an insurance denial, and understanding where the process is getting stuck can matter.
This is the patient side of the Medicare GLP-1 Bridge, including the twists, confusion, persistence, and lessons Dee Dee and Debbie learned while trying to get covered.

We Built a Free GLP-1 Support Community | Cozy Butter + On The Pen
Looking for a GLP-1 support group or community where you can connect with other patients without diet culture, spam, or an algorithm deciding what you see? We built one.
Cozy Butter is a free, patient-led GLP-1 community created by Kim Carlos of The Plus SideZ Podcast and Amanda Bonello of GLP-1 Studio. It was built for people taking or considering medications like Ozempic, Wegovy, Mounjaro, and Zepbound who want practical tools, education, peer support, events, and real conversations about living with obesity and metabolic disease.
We sat down with Dave Knapp of On The Pen to share how Cozy Butter started, how Kim and Amanda met through GLP-1 advocacy, and why we believe patients need more support between appointments.
We talk about GLP-1 peer support, obesity as a chronic disease, weight stigma, food noise, patient advocacy, health coaching, self-advocacy, GLP-1 tools, medication access, expert Q&As, community events, and what we hope to build next.

The World Has More GLP-1s but Who Can Afford Them? Part 2
This week’s GLP-1 news had us thinking about access. Oral Wegovy launched in Germany, and Hims & Hers expanded its GLP-1 services into Australia. But in the U.S., PepsiCo announced it would cut obesity medication coverage while 600,000 people signed up for the new Medicare GLP-1 Bridge program. That is less than 1% of all Medicare beneficiaries, although we still do not know how many were approved or received medication.
Four years after Mounjaro launched, are GLP-1s becoming easier to find but harder to get covered? It felt like the right time to revisit this Season 4 conversation with Dr. Michael “Mick” Crotty and Belinda Hogan about GLP-1 access, obesity care and weight bias around the world.
In Part 2, we discuss:
• Managing daily life and side effects on GLP-1 medication
• Why weight loss slows and plateaus happen
• How the body’s set point may affect weight
• What GLP-1 maintenance really means
• Balancing continued weight loss with stabilization
• Adjusting treatment for long-term care
• What Ireland could learn from the U.S. rollout of GLP-1 medicationsDr. Crotty is a GP, co-founder and clinical lead of My Best Weight in Ireland. Belinda shares her lived experience navigating GLP-1 side effects, plateaus, maintenance and long-term treatment.

GLP-1 vs. Big Food: Is the Food Industry Trying to Outsmart the Medications? What role has the food industry played in obesity, and how are GLP-1 medications changing what companies put on grocery store shelves?
Food scientist Liam Kincaid joins Kim and Bear to take us inside the science and psychology of food. They explore what makes certain foods easy to overeat, why novelty keeps us coming back, and whether sugar, preservatives, or any single ingredient deserves the blame.
Liam also explains why the grams of protein on a label do not tell the whole story, how to identify higher-quality protein sources, and why adding protein does not automatically make something “GLP-1 friendly.”
Plus, they discuss how GLP-1s can change cravings and food preferences, whether the food industry is trying to work around the medications’ effects, and why fiber, savory flavors, and entirely new products may shape the future of food.
This is an honest conversation about food, biology, access, and why people living with obesity deserve better than blame or another shiny label.

In Part 2 of Kat's conversation with Nanette Adams, MEd, LPC, LMHC, they have a candid discussion about eating disorders, substance use, and obesity care from both a professional and lived-experience perspective.
Nanette is a therapist specializing in obesity and eating disorders, an obesity advocate, and a person living with obesity who has experienced both bariatric surgery and GLP-1 treatment herself. Together, she and Kat explore how our relationships with food, movement, our bodies and other coping behaviors can change throughout obesity treatment, and why eating disorders can be particularly difficult to recognize when weight loss is being celebrated.
You'll leave with a better understanding of how eating disorders and substance use can intersect with obesity treatment, why they can be missed in higher-weight bodies, and when you or someone you love may need additional support.

_____________________________________________________________GLP-1 & Weight Regain: Why We Blame Ourselves
When weight regain happens, most people ask, "What am I doing wrong?" Weight regain can happen during GLP-1 treatment, after bariatric surgery, or years into an obesity journey. For many, the first response is self-blame.
Licensed therapist, obesity advocate, and person living with the disease of obesity, Nanette Adams, shares her personal journey while exploring why obesity is a chronic disease, how treatment often evolves over time, and the emotional toll of stalls, plateaus, and weight regain. We discuss therapy, shame, self-compassion, and why long-term support matters.

A Note Before You Listen: Our guest uses the term fat rather than obesity because that's the language of her community. At The Plus SideZ, we believe listening to different perspectives helps us better understand one another. If this language doesn't feel right for you, it's okay to honor that boundary and skip this episode.
The GLP-1 and body positivity communities are often portrayed as being at odds. But do they share more common ground than we realize?
In this episode, we sit down with Tigress Osborn, Executive Director of the National Association to Advance Fat Acceptance (NAAFA), to better understand the fat acceptance movement, the language it uses, and how it views obesity, GLP-1 medications, healthcare, body image, and weight stigma.
Together, we explore where these communities differ, where they overlap, and whether listening to one another can lead to greater understanding.
Whether you agree or disagree, our goal isn't to debate or sway. It's to listen, learn, consider a different perspective, and better understand one another.

How GLP-1s Broke Diet Culture
For decades, we believed weight gain and weight loss were mostly about willpower, calories, and exercise. Today, we’re learning there’s much more happening beneath the surface. The biology that drives hunger, metabolism, weight gain, and long-term health is far more complex than we once understood.
In this episode, Dr. Chiadi Ndumele, preventive cardiologist at Johns Hopkins Medicine and a leader with the American Heart Association, explains why GLP-1 medications work, how weight, metabolism, and chronic disease are connected, and why the first-ever Cardiovascular-Kidney-Metabolic (CKM) Clinical Practice Guideline represents a major shift in how we understand and treat weight-related disease. You’ll also hear a powerful patient story that shows why this new understanding is giving hope to millions.
In this episode:
• Why GLP-1 medications work when diets often don’t.
• What really drives weight gain, weight loss, and long-term weight management.
• How metabolism, hormones, genetics, and overall health work together.
• Why understanding your body can help you advocate for the quality care you deserve.Guest

Can GLP-1s Affect Food Noise, Alcohol, and Addiction?
** While Kim takes time away following the passing of her sister, we're revisiting some of our most impactful conversations. Thank you for your kindness, patience, and continued support of The Plus SideZ Podcast.
We're sharing this episode because it's more relevant today than when it first aired. Since then, research has expanded interest in whether GLP-1 medications may affect more than weight loss, including food noise, alcohol cravings, and other compulsive behaviors. While the science continues to evolve, so does the conversation.
Licensed therapist and addiction treatment leader Dr. Joy Bracey joins community member Brenda, who shares how Mounjaro changed her relationship with food, alcohol, and smoking. Together, they discuss the brain, behavior, and why this emerging area of research is drawing growing interest.
**In this episode:**
• Brenda's journey with obesity, binge eating, alcohol, smoking, and Mounjaro
• Food noise, binge eating, and emotional eating
• GLP-1 medications, dopamine, and the brain
• Alcohol cravings and addictive behaviors
• Mental health, self-love, and healing
• Why researchers are studying GLP-1 medications beyond weight loss*Originally recorded during Season 1. This episode reflects the information available at the time of recording. Research and clinical guidance have continued to evolve. It is intended for education and discussion only and is not medical advice. Always consult your healthcare provider regarding your individual care.*
