Non-Competes Are Kinda Bull$#!t
Show notes
We're about to make some employers mad. 😬
But here it goes: We think non-competes are bullshit.
And we're saying that as business owners who have spent years hiring, training, mentoring, and investing in our clinicians!
Why should getting a new job, starting your own practice, or leaving a workplace that isn't right for you mean having to move your family, change specialties, or drive an hour away just to earn a living?
In this 'sode, we're getting fired up about non-compete agreements and why we believe they're bad for employees, employers, AND the pelvic health profession.
We're talking about:
🚫 Why non-competes hurt our profession — How restricting clinicians' ability to change jobs limits opportunity, earning potential, and competition between employers.
⚖️ Are non-competes even legal? — Why the answer depends on your state, what happened to the proposed federal ban, and why understanding your specific agreement matters.
🤝 Non-competes vs. other employment protections — The important differences between non-competes, non-solicitation agreements, confidentiality, intellectual property, and training repayment agreements.
💪 You CAN negotiate your contract! — Why you shouldn't assume an employment agreement is set in stone, and what to consider before signing away your future options.
🏥 The employer perspective — We understand the fear of investing in an employee who eventually leaves. But is restricting their career really the answer?
🌱 Abundance vs. scarcity — Why great employers should focus on creating workplaces people WANT to stay at instead of trying to contractually force them to stay.
And yes, Jesse gets a little fired up. Nicole tries to keep things on track. We may or may not establish the official position of the newly formed Rising Task Force. 😂
Our bottom line? Your employer doesn't own your clinical skills, your degree, or your future.
If you're an employee, understand what you're signing and don't be afraid to negotiate.
If you're an employer, build a practice where people stay because they love working there—not because they're afraid to leave.
And remember, non-compete laws vary significantly by state. We're not attorneys, so make sure you understand the laws and agreements that apply to your specific situation.

Make Your Practice Feel Like a Bougie Hotel ⭐
Show notes
What if your pelvic health practice felt more like the Ritz-Carlton and less like a doctor's office? 🏝️
We're talking about how to create a five-star patient experience—and no, it doesn't mean you need to spend a fortune on fancy furniture or a massive clinic renovation!
Think about what makes a luxury hotel special. It's not just the expensive sheets or beautiful lobby. It's that every interaction is intentional. You feel welcomed, comfortable, remembered, and taken care of.
And that's exactly how we want our patients to feel.
In this 'sode, we're sharing practical ways to make your practice feel a whole lot more bougie:
🏨 The concierge welcome — Why the first 30 seconds at your front desk can immediately build trust (or create anxiety).
☕ It's the little things — Drinks, snacks, bed warmers, nice linens, and other inexpensive touches that make patients feel special.
🎵 Create the right atmosphere — How lighting, music, and personalized preferences can completely change the feel of your treatment room.
⏰ Punctuality is a luxury — Why running on time is one of the most important ways to show patients you respect them.
✨ The details matter — From perfectly prepared treatment rooms to thoughtful clinician handoffs, consistency is what separates good service from great service.
🎁 Go the extra mile — Small surprises, personalized touches, and making the final minutes of every visit memorable.
And we share plenty of examples from PelvicSanity—including a few things we STILL need to get better at (apparently our snack game needs some serious work 😂).
Here's the big takeaway: A premium experience isn't defined by how much money you spend. It's defined by how much thought you put into every interaction.
Whether you're a solo practitioner, own a multi-clinician practice, or work in a hospital system, there are things you can do THIS WEEK to make your patients feel more comfortable, valued, and cared for.
So grab your imaginary concierge bell, and let's make your practice a little more bougie! 🛎️

Are We Teaching Birth Prep Wrong? With Jen Hamilton
Show notes
Jen Freaking Hamilton is on the podcast!! 🎉
With more than 6 MILLION followers across social media, a #1 New York Times bestselling book (Birth Vibes), and 15 years of nursing experience, Jen Hamilton has become one of the most influential voices in birth education.
And she's joining us for a conversation that every pelvic PT and OT who works with pregnant patients needs to hear.
Because here's the uncomfortable question: Are we sometimes setting our patients up for disappointment with the way we teach birth prep?
We talk about:
🤰 When birth plans backfire — How rigid expectations around pushing, positioning, and interventions can leave patients feeling like they've failed when birth doesn't go according to plan.
💪 The truth about pushing positions — Why there's no single "right" way to give birth, and what Jen wishes pelvic rehab providers understood about the realities of the delivery room.
🧠 Are we overcomplicating birth prep? — Why giving patients a giant checklist of techniques and positions might actually make things harder when labor gets intense.
❤️ Trust isn't automatic. It's earned. — Jen's incredible approach to helping patients feel safe, heard, and empowered—even when they're initially distrustful of their medical team.
🤝 Doulas, advocacy, and informed consent — How to help patients advocate for themselves without creating an adversarial relationship with their birth team.
🚨 When emergencies happen — What compassionate communication looks like when every second counts.
📖 The philosophy behind Birth Vibes — Why Jen wants patients to prepare for how they want to experience birth, not just exactly how they want it to happen.
Nicole also shares her own birth experience—and how even nearly 20 years as a pelvic PT didn't prepare her for everything that happened in the delivery room.
The big takeaway? Our job isn't to prepare patients for one perfect version of birth. It's to help them feel confident, informed, and supported no matter how their birth unfolds.
This one might challenge some of what you've been teaching. And that's exactly why we wanted to have this conversation!

Preventing the December Slowdown in Business
Show notes
Every year, pelvic health business owners start dreading the same thing: the dreaded December slowdown. 🎄
Patients are traveling. Family commitments pile up. People cancel or reschedule appointments. And sometimes we're closing our own doors for the holidays!
But here's the question: Does December actually have to be a bad month for your business?
We don't think so!
Every month has its challenges AND its opportunities. And December has some pretty big advantages: patients want to reach their goals before the new year, use up their FSA dollars, and often experience symptom flares with holiday stress and travel.
In this 'sode, we're sharing 3 ways to prevent the December slowdown before it starts:
🩺 1. It starts in the treatment room. Use the Last 5 minutes of every session to discuss holiday plans, upcoming travel, and the importance of consistency. Don't wait until patients start cancelling to have these conversations!
📅 2. Get ahead of scheduling. Your front desk should be booking patients through January, not treating December 31st like the finish line for their care. With holiday vacations and limited availability, every appointment becomes even more valuable.
📣 3. Don't stop marketing! So many practices pull back their marketing in November and December—right when they need it most. The new patients you're bringing in NOW are the ones who will help keep your schedule full through the holidays.
The December slowdown isn't something that just happens to you. It's something you can anticipate, plan for, and actively work to prevent.
And the time to start is NOW!
🚀 Want a Business That Thrives All Year?
Inside the Pelvic PT Rising Accelerator Program, we help pelvic health business owners build the systems, marketing strategies, and patient management processes that create a more profitable, predictable, and sustainable practice.
Because you shouldn't have to cross your fingers and hope your schedule stays full—or feel like you're starting from scratch every January.

26 Years of Pelvic Health Lessons: An Interview with Tracy Sher
Show notes
What does 26 years of treating complex pelvic pain teach you? And what happens when you decide it's time to step away from patient care?
We're joined by Tracy Sher, founder and CEO of Pelvic Global (formerly Pelvic Guru), international educator, and one of the pioneers who helped shape how our field approaches complex pelvic pain.
This conversation is packed with clinical pearls, honest reflections, and some incredible stories about building a business (including a VERY expensive picture of a uterus 😳).
We dive into:
⏳ "How long until I get better?" — Tracy's brilliant framework for answering one of the hardest patient questions without making promises you can't keep.
📈 Redefining progress — Why improvement isn't always about pain scores or symptoms disappearing, and how to help patients recognize meaningful wins.
🧠 Anxiety isn't the same as pain — How Tracy uses separate scales to guide examinations, build trust, and help patients feel safe.
🤝 Better mentorship, better referrals — Why pelvic PTs and OTs should be learning from each other and referring within our own specialty more often.
🌎 Lessons from around the world — What Tracy has learned teaching internationally, and why complex pelvic pain education still has a long way to go.
💸 The $20,000 uterus picture — How a copyright lawsuit, failed business ideas, and some expensive mistakes helped shape Pelvic Global.
❤️ Stepping away after 26 years — The emotional side of leaving direct patient care when being a clinician has been such a huge part of your identity.
One of our favorite takeaways? You don't need to know exactly when a patient will get better. But you do need a plan for figuring out whether you're on the right track.
And after 26 years, Tracy is still curious, still learning, and still finding new ways to make an impact on our field.
About Tracy Sher
Tracy Sher is a pelvic health physical therapist with 26 years of clinical experience, an international educator, and the founder and CEO of Pelvic Global, formerly Pelvic Guru.
She's known for her expertise in complex pelvic pain, including pudendal neuralgia, and for creating educational resources that help clinicians around the world deliver better care.
She's also the creator of The Art of Pelvic Anatomy, a beautifully illustrated anatomy resource designed for clinicians, educators, and patients.

You Built the Business. Now Can You Step Away?
Show notes
What happens when you hit the financial goals…but realize you still can't slow down?
This episode was sparked by a business owner who had been in practice for five years, was financially successful, but felt like she still couldn't step back, stop grinding or really enjoy what she'd built.
It got us thinking about the three very different seasons of PelvicSanity over the last 10 years—and how our definition of a successful business has changed along the way.
In this episode, we talk about:
🚀 Season 1: Growth — 80-hour weeks, rapid growth and proving the business could actually work.
🧑💼 Season 2: Leadership — Learning (sometimes the hard way) that doing everything ourselves wasn't sustainable—and that leading by example isn't actually enough.
⏰ Season 3: Time Freedom — Deliberately building a business that no longer depends on us being there every day.
🏝️ Why we regret not taking more vacations during the early years—and don't believe working every Saturday was responsible for our success.
📈 The realization that how much the business makes isn't directly tied to how much we work.
🧠 Why after years of being rewarded for grinding, stepping back is a skill you have to learn.
✋ And why sometimes you can't wait until you feel ready to slow down. You have to create the boundary first, see what breaks, and then build the systems to fix it.
Your business will go through seasons. The way you built it doesn't have to be the way you run it forever.
And if you've realized the business you've created isn't giving you the life you actually want, that doesn't mean building a business was a mistake. It means it's time to build the next version of it.
Ready for Your Next Season?
If your business is successful on paper but still requires too much of you, that's exactly the kind of problem we work on inside the Pelvic PT Rising Accelerator.
We'll help you build a more profitable, sustainable business—with the systems, strategy and support to create more freedom, not just more work.

Does Your Business Get Easier Each Year? Nicole vs. Jesse Debate!
Show notes
The longer you own a business…the easier it should get, right?
Well, Nicole and Jesse aren't so sure they agree. 😂
So we're debating it!
📈 On one hand, YOU get better every year. You know more. You've made mistakes. You've survived hard things. Problems that would have sent you spiraling five years ago barely register anymore.
But…
🔥 Your problems get bigger.
😬 You fall into bad habits.
🏃♀️ It gets harder to maintain momentum.
🤯 And as the business grows, you encounter entirely new challenges you never had when you were starting out.
So does business ownership actually get easier with experience?
Or does it just get…differently hard?
We make the case for both sides—and talk about the thing that has probably done more than anything else to make entrepreneurship easier and more enjoyable for us: coaching.
We've learned a LOT from our own mistakes over the years. But we've learned even more by shortening the learning curve, getting outside perspective, and benefiting from mistakes other smart business owners have already made.
Because you certainly can figure everything out yourself.
But you don't have to. 🙌
🚀 Want Business to Get Easier?
That's exactly why we created the Pelvic Health Business Accelerator.
You don't need to reinvent the wheel, make every mistake yourself, or spend years figuring out lessons someone else could help you see in minutes.

5 Business Lessons From 5 Years of PelviCon
Show notes
PelviCon is officially here! 🎉
Five years ago, PelviCon was basically an audacious idea: What if we built the pelvic health conference we always wished existed?
Since then, we've sold out five years in a row, grown to more than 600 in-person attendees, built an incredible Live Virtual experience—and learned a LOT about business along the way.
So we're pulling back the curtain on some of the biggest lessons PelviCon has taught us:
💡 Build what you wish existed. Don't just make something slightly better. Ask whether you can create something meaningfully different.
🧪 If you're going to fail, fail early. Test the idea, start small and get actual proof before making the giant investment.
🛑 Don't dilute what makes you special just to grow. We could fit more people into PelviCon. We've deliberately chosen not to when it would compromise the experience.
📘 Buy the color manuals. Figure out the little things that define your brand—and don't nickel-and-dime them away just because you could save money.
🤝 Partnerships are hard—and alignment matters. PelviCon has shown us both why we're usually cautious about partnerships and what can make a great one actually work.
⭐ Only work with great people. Great speakers. Great AV. Great partners. Great vendors. Life and business get a whole lot better when you stop settling for “good enough.”
And maybe the bonus lesson: everyone has an opinion. Listen carefully to your customers, take feedback seriously—and still be willing to protect your own vision.
We're recording this as PelviCon kicks off, so we're going to stop talking business and go enjoy the thing we've spent five years building. If you're joining us in Atlanta or Live Virtual...let's freaking go! 🔥

There’s No Such Thing as a Simple Pelvic Health Patient: An Interview with Michelle Lyons
Show notes
There’s no such thing as a simple pelvic health patient.
Even the patient who walks in with “just a little stress incontinence” brings a history, a body, hormones, sleep, stress, previous injuries, bowel function, activity goals—and a whole lot more.
And that's exactly why being a great pelvic health clinician requires so much more than memorizing protocols.
We're joined by Michelle Lyons for one of those wide-ranging, thought-provoking conversations we always love having with her.
Michelle has been part of PelviCon from the very beginning. She's our only 5-time PelviCon speaker, an incredible educator and one of the most thoughtful voices in our profession.
In this episode, we talk about:
🪟 Why assessing the pelvic floor still matters. Michelle shares the analogy of an internal + external assessment being like “washing the windows on both sides”—and why telehealth can be valuable as well as, rather than instead of, in-person assessment.
🧩 Why there are no simple patients. Even seemingly straightforward incontinence can require us to think about hips, hormones, sleep, iron, constipation, previous injuries, activity and more.
🧠 What clinical reasoning actually looks like. Information isn't enough. Great clinicians have to connect seemingly unrelated pieces into a coherent picture.
🎓 Why pelvic health desperately needs more mentorship. Taking Pelvic Floor 1 doesn't magically make someone an expert. New clinicians need support, supervision and safe places to ask questions.
🔬 Why better research isn't enough. Michelle used to believe better research would automatically lead to better pelvic health care. She's changed her mind. We also need to get much better at translating evidence into actual behavior change.
🌱 Why clinicians are facilitators, not fixers. Our job is to create the conditions where patients can recover, build capacity and thrive.
And Michelle gives us a preview of her two PelviCon talks—one on pelvic health after cancer and another on women and back pain—including some brand-new oncology guidelines she's been involved in developing.

Your Business Doesn't Have to Grow
Show notes
More revenue. More patients. More clinicians. More locations.
We're constantly told that a "successful" business should always be getting bigger.
But what if you don't actually want it to? 🤔
There are plenty of GREAT reasons not to grow.
👨👩👧👦 You're in a season where family needs more of you.
✈️ You want to travel or prioritize life outside the practice.
💰 You're already making enough money.
🙅♀️ You have absolutely zero desire to manage employees.
❤️ Or maybe you're just…happy with what you've built!
And sometimes growth stalls for reasons you didn't choose—staff turnover, seasonality, capacity issues, losing a referral source, or just the normal ups and downs of running a business.
A flat year doesn't mean you failed.
But there's an important flip side…
"I don't want to grow" can also become a really convenient story we tell ourselves when we're actually afraid to grow. 😬
There's a difference between intentionally choosing to maintain your business and avoiding the hard things required to take the next step.
And remember: not growing doesn't mean not improving.
If your revenue stays flat but you work 10 fewer hours a week, take four more weeks of vacation, increase your profit, improve your systems and enjoy your business more?
🎉 That's a better business.
The question isn't:
"Did my business grow this year?"
It's:
👉 "Did my business do what I wanted it to do for me this year?"
🚀 Want to build a business that actually works for YOU?
That's exactly what we do in the Pelvic Health Business Accelerator—helping pelvic health business owners get consistently full, improve their systems and build the business they actually want.

How to Actually Be an Evidence-Based Clinician: An Interview with Taryn Hallam
Show notes
Reading research and actually understanding what it means for the patient in front of you are two VERY different things.
And there may be nobody in pelvic health better at bridging that gap than Taryn Hallam.
Taryn has essentially made research translation her full-time job—reading the enormous amount of pelvic health research being published and asking:
What does this actually mean in the clinic?
Because the abstract doesn't always tell the whole story.
As Taryn puts it, the abstract is like the trailer for a movie. Sometimes the trailer accurately represents the movie. And sometimes...all the best parts were in the trailer. 😆
In this episode, we're diving deep into how we actually become evidence-based clinicians.
We talk about:
📊 Statistical significance vs. clinical significance. A result can be completely “statistically significant” and still be basically meaningless to your patient.
🎬 Why you can't just read the abstract. What gets highlighted in an abstract isn't necessarily what matters most clinically.
🔬 Why one research paper is never the whole story. Taryn compares a single paper to opening a 450-page novel to page 210 and expecting to understand the plot.
🤰 The danger of extrapolating research to the wrong population. What happens when research on young, nulliparous athletes gets applied to a completely different patient sitting in front of us?
🤷♀️ Why “we don't know” is sometimes the most evidence-based answer. We don't need to pretend the research is more definitive than it actually is.
🧠 How experience changes our ability to assess. Can research showing poor reliability among relatively inexperienced clinicians really tell us what an expert with decades of pattern recognition can—or can't—feel?
🔥 And yes...we ask Taryn about the internal exam debate.
Her answer?
Let's just say she has some thoughts. 😆
We talk prolapse, stress urinary incontinence, levator avulsion, pelvic floor muscle training, tone, overactivity—and why Taryn believes an internal examination remains an essential component of best-practice pelvic health care.
But she also makes an incredibly important distinction:
Saying virtual care has value and saying an internal examination adds important clinical information are NOT contradictory statements.
Both can be true.
And sometimes our profession gets into arguments because we haven't even agreed on what question we're debating.
🎟️ Learn From Taryn at PelviCon 2026!
If you like Taryn on the podcast, you are going to LOVE her at PelviCon.
She's bringing us two completely different—and appropriately disruptive—presentations.
Should Possible Levator Avulsion Change Our Approach?
This one is for the research nerds. 🤓
Taryn is digging into the actual studies and statistics, including what odds ratios and p-values really tell us, to ask whether suspected levator avulsion should change how we approach someone in those first few months postpartum.
Coital Incontinence & Female Ejaculation
And this one tackles a topic many of us simply weren't taught enough about.
Our patients may be nervous enough to bring these concerns up. We shouldn't be sitting there thinking, Oh crap...what do I say now? 😬
Taryn is going to give us the research, understanding and language to have these conversations with confidence.

What’s the Point If You Hate Your Business?
Show notes
You can make great money. You can have control of your schedule.
But…do you actually like the business you've created? 😬
In Part 3 of our Make the Business Work for You series, we're talking about the third major piece of upside you traded for when you became a business owner: satisfaction & impact. 🙌
Ideally, as your business grows, your role should get BETTER.
✨ More time doing things you're great at.
❤️ More of the work you genuinely enjoy.
💰 More time spent on things that actually create value.
🗑️ Less answering phones, chasing paperwork, putting out preventable fires, scrambling for patients, and doing all the little things you hate.
And then there's impact.
Maybe success means becoming the go-to pelvic health resource in your community. Maybe it's mentoring clinicians, creating incredible jobs, building relationships with providers, or growing a team that can help far more patients than you ever could by yourself. 🌎
There's no single "right" version of a successful pelvic health business.
And that's the whole point of this series.
Money. Time. Satisfaction & impact.
You don't have to maximize all three at the same time—and sometimes you can't.
But you gave up a LOT of consistency to become an entrepreneur.
👉 Make sure you're actually collecting the upside you traded for.
🚀 Want to build a business that actually works for YOU?
That's exactly what we do in the Pelvic Health Business Accelerator—helping you build a thriving practice that's financially successful without losing sight of the life and career you actually wanted when you started.

Are We Really Providing Whole-Person Care If We Ignore Spirituality? With Sinéad Dufour
Show notes
We talk a lot about providing “whole-person care.” But are we actually doing it?
And if we leave spirituality completely out of the conversation...can we really call our care whole-person?
This is exactly the kind of question we love exploring with Dr. Sinéad Dufour.
Sinéad is one of the researchers and educators we respect most because she's constantly willing to examine the evidence, challenge her own assumptions and change her mind when the science takes her somewhere new.
So naturally, when she decided to go back to graduate school for counseling and spiritual care, we knew the conversation was going to be fascinating. 😆
And it's the foundation for the panel she's leading at PelviCon this year:
Spirit in Session: Towards Whole-Person Pelvic Healthcare
In this episode, we dive into:
✨ Spirituality vs. religion — why they're not the same thing, and why that distinction matters in healthcare.
🧠 Is biopsychosocial care actually whole-person care? Sinéad argues there's another domain we often leave out.
❤️ Purpose, meaning and compassion in the treatment room — and how pelvic health clinicians may already be touching on spirituality without realizing it.
🚧 Where's the line between whole-person care and “out of scope”? And what training, mentorship and self-awareness do clinicians need?
💻 What does GOOD virtual pelvic health care actually look like? Sinéad has built both in-person and virtual practices and has some fascinating thoughts about where virtual care shines—and where it simply cannot replace in-person assessment.
👀 The great irony of virtual care: Sinéad believes that becoming excellent virtually may require years of honing your skills in person first.
🤝 Why pelvic PTs need to refer to other pelvic PTs more often. Sometimes the best care isn't choosing virtual or in-person—it's creating a circle of care around the patient.
And Sinéad is now leading research to answer one of the questions our profession desperately needs better evidence around:
What actually constitutes high-quality virtual pelvic health care?
We can't wait to see what she finds.
🎟️ Learn from Sinéad LIVE at PelviCon 2026!
We're now just DAYS away from PelviCon!
Sinéad will be leading what might be one of the most unique conversations we've ever had on the PelviCon stage:
Spirit in Session: Towards Whole-Person Pelvic Healthcare

Make the Business Work For You - Time Freedom
Show notes
You started your business for more freedom.
So…why are you working more hours, answering messages on vacation, staying late for the one patient who "can only do 5pm," and feeling guilty anytime you need to change your schedule? 😬
In Part 2 of our Make the Business Work for You series, we're talking about TIME FREEDOM 🙌
⏰ How much time does your business actually require from you?
📅 Are you working the schedule you want—or the schedule your patients have created for you?
🧹 How much timesucking nonsense is quietly stealing hours from your week?
🤯 And if your kid gets sick, you want to take a vacation, or life just happens…do you actually feel free to change your schedule?
We also get into why having 28 appointment openings when you're only seeing 12 patients is a recipe for feeling like you're always at work—and why your patients are probably WAY more flexible than you think.
Because there's nothing worse than leaving a job for more freedom…and discovering you've become a more demanding boss than your old employer ever was. 😳
You traded consistency for upside.
Time freedom is part of that upside. Don't give it away. 💪
🚀 Want to build a business that actually works for YOU?
That's exactly what we do in the Pelvic Health Business Accelerator—help you get consistently full, eliminate the nonsense, build better systems, and create a business that supports the life you actually want.

Disrupting Sexual Health with Dr. Rachel Rubin
Show notes
Dr. Rachel Rubin is changing the landscape of women’s health—and she’s not waiting around for permission.
In this fiery conversation, we talk about why sexual medicine has been ignored for so long, the massive education gap around menopause and hormones, and Rachel’s work fighting to remove the FDA boxed warning from vaginal estrogen.
We also get into why pelvic PTs and OTs need to own our role in recognizing hormonal and vulvar changes, why the future of women’s healthcare requires a multidisciplinary “pit crew,” and Rachel’s philosophy that no grown-ups are coming to save the day.
Plus, we talk disruption, entrepreneurship, taking action, dealing with criticism…and why Rachel believes pelvic rehab providers are becoming leaders in this space.

Make the Business Work For You - Financial Success
Show notes
When you leave a job to start your own practice, you're making a pretty massive trade:
You're trading consistency for upside.
Gone is the predictable paycheck, the regular schedule, the structure someone else created for you, and the relative certainty of knowing what you're going to make.
In exchange? You get upside.
And in this first episode of our Making Your Business Work for You series, we're talking about the most obvious kind: money.
Because we all know the fantasy math.
"If I charge $200 and see 40 patients a week…"
And suddenly you're mentally spending the fortune you're obviously going to make as a practice owner.
Except…that's not how this works.
There are plenty of practice owners making less than they could working for someone else. We'll share some real numbers, break down what different caseloads actually mean for your take-home income, and talk about the financial ceiling of the solo model versus the very different challenges (and opportunities) of growing beyond yourself.
We also break down the only four ways to make more money in your practice:
• Get more leads
• Convert more of those leads into patients
• Keep your patients longer
• Raise your pricesBut making more isn't the whole story.
You can have a thriving practice and still panic over every cancellation, obsess over every eval who doesn't book, and wonder whether the whole thing is about to come crashing down after one slow week.
So we're also talking about financial confidence—knowing your numbers, understanding the normal ups and downs of business, and being confident you're on pace to hit your goals.
Because ultimately, running a small business is a get-rich-slow scheme.
You stack the wins. You make an extra $25,000 this year. You invest it. You get smarter about taxes. You improve the business. You let those gains compound over years and decades.
Your practice might be the single greatest wealth-generating machine you ever create.
Just don't expect it to happen overnight.

You Know His Research. But Do You Know Paul Hodges?
Show notes
You might not immediately recognize the name Paul Hodges. But we can almost guarantee you've been taught his work.
And if you're attending PelviCon (either in-person or with our Live Virtual option) you'll get the chance to learn from him directly!
If you've learned that...
👉 The pelvic floor and diaphragm work together
👉 The pelvic floor is part of a larger pressure-management system
👉 Timing and coordination matter—not just strength
👉 The abdominals and pelvic floor don't function independently
👉 Intra-abdominal pressure isn't inherently bad
👉 Pain is far more complicated than simply "tissue vs. brain"
...then you've been influenced by research Professor Paul Hodges and his collaborators have spent decades developing.
And that's why we were SO excited to sit down with him.
Paul is one of the most influential physiotherapy researchers in the world, and in this episode we get to nerd out with him about the science that has fundamentally shaped how we practice.
We talk about:
🔬 What we got wrong about his early motor-control research — and how complex science gets oversimplified when it reaches the clinic.
💨 Why pressure isn't the enemy. Paul introduces the Swedish concept of lagom: we need the appropriate amount of pressure for the task—not simply less pressure.
♂️ Why we can't just extrapolate female pelvic floor rehab to men. Paul's team has been investigating what actually creates continence after prostatectomy and how that should change rehabilitation.
🧠 Why we've oversimplified chronic pain. We dig into nociceptive, neuropathic and nociplastic pain—and why chronic pain doesn't automatically mean the tissues are irrelevant.
🤯 The problems with “central sensitization.” Including Nicole getting a little surprise when Paul explains the limitations of the Central Sensitization Inventory!
📚 How research actually makes its way into clinical practice—and why Paul believes research should ultimately change outcomes for real people.
And maybe our favorite theme of the entire conversation:
Complexity doesn't mean we throw our hands up.
Our job is to understand that complexity well enough to find the clinically meaningful pieces that help the person sitting in front of us.
🎟️ Learn From Paul LIVE at PelviCon 2026!
If this conversation gets your clinical brain firing, just wait until PelviCon. (You can still get your tickets at www.pelvicon.com/virtual)

Professor Paul Hodges is a physiotherapist, neuroscientist and professor at the University of Queensland whose research has profoundly influenced our understanding of motor control, biomechanics, pain, continence, breathing and pelvic floor function.
His work spans decades and hundreds of scientific publications, but what makes it so relevant to us is how many of the principles we now take for granted in pelvic rehab have roots in questions Paul and his collaborators were asking long before those concepts became commonplace.

Do You Actually Need a CRM? Fight for Simplicity.
Show notes
When does a cash-based pelvic health practice actually need a fancy CRM?
For most of us? Never.
And yes—we've made this mistake ourselves. 😬
In this episode, we're breaking down what a CRM actually is, how it's different from your EMR and email platform, and why adding more technology often creates more problems instead of solving them.
We get into:
👉 Why you probably already have a CRM (even if it's just a Google Sheet!)
👉 The difference between your CRM, EMR and email platform
👉 Why PelvicSanity went from a simple Call Log → fancy CRM → right back to the Call Log
👉 The hidden costs of tech stacking and automation
👉 Why you shouldn't build your business systems around a specific admin
👉 The question you should ask before adding ANY new software, system or complexity
The bigger principle?
Fight for simplicity.
Before adding something new to your business, get crystal clear: What problem am I actually trying to solve?
Then ask whether this is really the simplest, best way to solve it.
Because in more than 1,000 pelvic health business owners we've worked with, we've never once thought:
"You know what's really holding this business back? They need a better CRM." 😆
🚀 Want Our Actual Systems? Join the Accelerator!
This is exactly the kind of stuff we work through in the Pelvic Health Business Accelerator.
You don't need to reinvent your systems or spend months testing fancy software.

Maternal Mortality, a Whole-Body Approach and Using Data: An Interview with Katherine Sylvester
Show notes
How can we help save the lives of our pregnant and post-partum patients? What about doing more than just resolving their reported symptoms?
We can't wait for you to hear this conversation with Katherine Sylvester - she has spent her entire career challenging the status quo and pushing for better results for her patients.
After experiencing postpartum preeclampsia herself, Katherine began asking how wearable data could help women recognize problems earlier, advocate for themselves and make better decisions during pregnancy and postpartum. That eventually grew into Operation M.I.S.T. and her work helping women use their own data to better understand their bodies.
And that's just one part of this conversation!
We talk about:
👉 How Katherine's own postpartum experience led to Operation M.I.S.T.
👉 Using wearable data to identify concerning changes before they become major problems
👉 Teaching women to self-monitor and make data-driven decisions
👉 Why postpartum return to exercise needs to be individualized
👉 Her mission to reduce unnecessary C-sections
👉 Why we're barely scratching the surface of what pelvic PT/OT can do with scar management
👉 What happens when you stop complaining about problems and start asking, “What can I do about this?”
Katherine is bringing two completely new topics to the PelviCon stage this year: using data to address maternal mortality and better understanding and treating scars in pelvic rehab. We can't wait for you to learn from her!

The Hardest Part of Hiring Your First Clinician
Show notes
What if the first clinician you hire isn't as good as you?
It's one of the biggest fears we hear from successful solopreneurs—and it's exactly what Nicole struggled with when we started growing PelvicSanity.
Because hiring your first clinician isn't just adding another person to the schedule.
It's changing your entire role.
Suddenly, you have to trust someone else with your patients, your reputation, your culture and something you've worked incredibly hard to build.
And yes...Nicole struggled with this enough that she eventually went to therapy to work through some of the control issues that came with becoming a leader. 😬
In this episode, we're talking about the mindset shifts required to successfully go from solopreneur to multi-clinician owner, including:
👉 Why being busy isn't a good enough reason to hire
👉 "No one will ever be as good as me" (and what to believe instead)
👉 Why your next clinician can actually enhance your reputation
👉 Raising the floor with great systems, onboarding and mentorship
👉 Raising the ceiling by helping your clinicians become incredible
👉 Why your clinicians don't need to be perfect on Day 1
👉 The fear that you'll invest in someone...and they'll eventually leave
Ultimately, the traits that made you a successful solopreneur—control, high standards, personal responsibility—can become the exact things that prevent you from building a great team.
Growing beyond yourself requires becoming a different kind of business owner.
